Forms for Disclosing Donations (Members and MEPs)

The following forms have been produced by the Standards Commission for use by TDs, Senators and MEPs in making their annual disclosures under section 24 of the Electoral Act 1997, as amended.  Explanatory notes on the completion of the forms are also provided.  Completed forms must be returned on an annual basis to the Standards Commission by 31 January.  The forms and explanatory notes are available in PDF and MS Word format.
